The museum was founded based on the private collections of Kazimierz Maria and Tadeusz Osiński, members of the local society, which were later enriched by numerous donations from Przemyśl residents and the surrounding population. Over the years, the collections grew through purchases as well as field research and explorations. The Osiński brothers gathered artifacts in the fields of archaeology, ethnography, art, historical memorabilia and archival materials. Before 1909, the collection included over 1,500 objects, about 3,000 books, and approximately 2,000 town and land records.

The official inauguration of the museum took place on April 10, 1910, with the participation of numerous city and church officials as well as local residents. During the ceremony, Bishop Józef Sebastian Pelczar gave a speech, blessing the institution and wishing for its development for the benefit of Polish science and culture.

In 1921, at a museum conference in Poznań, the institution was named the National Museum of the Przemyśl Land and became one of 14 Polish museums admitted to the Union of Historical and Artistic Museums.

Today, the museum is a multi-departmental institution with collections in archaeology, ethnography, history, art and crafts, and natural history. The library holds over 36,000 volumes of books and periodicals. Within the organizational structure of the National Museum of the Przemyśl Land, there are two branches: the Museum of Bells and Pipes (opened in 2001) and the Museum of the History of Przemyśl (opened in 2005).
